Looking After Me – a course for carers

Calderdale Carers Project run a course called Looking After Me – it’s designed specifically for carers and offers all kinds of useful and important information and advice. The course has been running for several years now and continues to gain popularity – it really is a unique opportunity for carers, who are often stressed and/or isolated, to get together and learn together how to look after themselves and get everything they are entitled to as a carer.

Health Walks in Calderdale January‐March 2012

Health walks are a great way to be more active, improve your health and make new friends. They’re not about rambling, hiking or ‘power walking’ ‐ they’re just short led walks in your local area at your own pace.

All our walks are led by qualified, friendly walk leaders who will welcome you to the group and support you on your walk. Click here to download the programme as a PFD.

For any enquiries about Walk It Together please call 01422 281529 or email walkit@swyt.nhs.uk.

Gateway to Care – new improved and additional services

0845 111 1103

One number to call for adult health and social care support needs

People can now call Gateway to Care for adult health and social care needs and for help to get back independence, mobility and confidence after an accident, illness, injury or deterioration in a condition.
The improved service was set up after the Council and NHS Calderdale asked patients, the public and staff about the kind of services they would like.
People said that they wanted a way to get help and advice quickly and easily, with one assessment. People also said they wanted to avoid being admitted to hospital and wanted services to work together to care for them safely at home instead.
To support the additional service, Gateway to Care is now staffed by dedicated nurses, social workers and trained advisors who are working together to make sure people receive coordinated care, help and advice.
Call Gateway to Care on 0845 111 1103 to speak to an advisor or email your enquiry to gatewaytocare@calderdale.gov.uk
The service is open from 8:45am to 5:30pm, Monday to Thursday and 8:30am to 5pm on Fridays. Out of hours arrangements will apply at all other times.
If people would like copies of the posters, leaflets or information cards about the service, please contact Nicola King from the project team on 01422 281431 or email nicola.king@calderdale.nhs.uk
